These State-Specific Notices supplement the Terms of Service, Telehealth Informed Consent, Notice of Privacy Practices, and Privacy Policy. They apply only where required by applicable law.
California
Notice to Patients
Medical doctors are licensed and regulated by the Medical Board of California. To check up on a license or to file a complaint, visit the Medical Board of California, email licensecheck@mbc.ca.gov, or call (800) 633-2322.
Notice to Consumers — Osteopathic Physicians
Osteopathic physicians and surgeons (D.O.) are licensed and regulated by the Osteopathic Medical Board of California. For license verification or to file a complaint, visit the Osteopathic Medical Board of California.
Open Payments
For informational purposes only, a link to the federal CMS Open Payments website is provided. The federal Physician Payments Sunshine Act requires that information concerning certain payments and other transfers of value made by manufacturers to physicians and teaching hospitals be made available to the public via the CMS Open Payments Database.
Colorado
Complaints regarding Colorado-licensed healthcare professionals may be submitted through the Colorado Department of Regulatory Agencies – File a Complaint.
District of Columbia
Complaints regarding District of Columbia licensed healthcare professionals may be submitted through the District of Columbia Board of Medicine.
Florida
Complaints regarding Florida-licensed healthcare professionals may be submitted through the Florida Department of Health – File a Complaint. If the Medical Practice offers services subject to the Florida Weight-Loss Consumer Bill of Rights, all applicable consumer disclosures will be provided in accordance with Florida law.
Idaho
Complaints regarding Idaho-licensed healthcare professionals may be submitted through the Idaho Division of Occupational and Professional Licenses.
Illinois
Complaints regarding Illinois-licensed healthcare professionals may be submitted through the Illinois Department of Financial and Professional Regulation – File a Complaint.
Indiana
For professional licensing information or to file a complaint, visit the Indiana Attorney General Licensing Enforcement Unit.
Iowa
For professional licensing information or to file a complaint, visit the Iowa Department of Inspections, Appeals, and Licensing.
Kentucky
Complaints regarding Kentucky-licensed healthcare professionals may be submitted through the Kentucky Board of Medical Licensure – File a Complaint.
Maine
Complaints regarding Maine-licensed healthcare professionals may be submitted through the Maine Board of Licensure in Medicine – File a Complaint.
Maryland
Complaints regarding Maryland-licensed healthcare professionals may be submitted through the Maryland Board of Physicians – File a Complaint.
Nebraska
Complaints regarding Nebraska-licensed healthcare professionals may be submitted through the Nebraska Department of Health and Human Services – File a Complaint.
Oklahoma
For medical doctors, visit the Oklahoma State Board of Medical Licensure and Supervision. For osteopathic physicians, visit the Oklahoma State Board of Osteopathic Examiners.
