Your Right to a Good Faith Estimate

Under the federal No Surprises Act, you have the right to receive a written "Good Faith Estimate" of expected charges from your healthcare provider before you receive non-emergency services. This notice explains how that right works at Yuvia.
1. What a Good Faith Estimate is
A Good Faith Estimate ("GFE") is a written estimate of expected charges for a healthcare service, provided before the service is rendered. It is required for patients who are uninsured or who choose not to use their insurance ("self-pay" patients). The GFE helps you understand the cost of care in advance and decide whether to proceed.
2. Who receives one
Yuvia is a cash-pay-only telehealth platform. We do not bill insurance. Every Yuvia patient is a self-pay patient under the No Surprises Act, and therefore entitled to a Good Faith Estimate before any prescription is issued or fulfilled.
3. When you receive yours
You receive a Good Faith Estimate inside your medical intake, before your physician issues any clinical decision and before any charge is placed on your payment method. The estimate is displayed on-screen and emailed to the address on your account. You may also download a copy at any time from your patient portal.
4. What's included
Your Good Faith Estimate includes:
- The product or protocol you've selected and the dosage form.
- The monthly subscription price.
- The expected duration of treatment, if applicable, and the total expected charges if you continue for that duration.
- Any one-time fees (none currently apply at Yuvia).
- A line item for shipping (currently included in the subscription price).
- The names of the provider, the compounding pharmacy that will fulfill, and Yuvia Holdings LLC
- Notice of your right to dispute the bill if actual charges exceed the estimate by $400 or more.
5. Our published pricing
Yuvia publishes its monthly subscription prices on the product reveal screens of our marketing quizzes and on the checkout page. Prices in effect at the time you submit your intake are the prices on your Good Faith Estimate. We do not charge consultation fees, dispensing fees, or any line item beyond the monthly subscription price for the prescribed product.
6. No charge until approval
Yuvia uses a Stripe Setup Intent at checkout, which authorizes your payment method without charging it. $0 is due today. Your card is charged only after a licensed physician reviews your intake and approves your prescription. If the physician declines, your card is never charged. You may cancel your subscription at any time from the patient portal; cancellation is effective immediately and no further charges will be placed.
7. Your right to dispute
If you receive a bill from Yuvia that is $400 or more above the total charges listed in your Good Faith Estimate, you have the right to dispute the bill under the No Surprises Act. You must initiate a dispute within 120 days of the date on the bill.
The federal Patient-Provider Dispute Resolution ("PPDR") process is administered by the U.S. Department of Health and Human Services. A decision from the independent reviewer is binding on both you and Yuvia.
8. How to dispute
To dispute a bill that exceeds your Good Faith Estimate by $400 or more:
- Visit the federal Patient-Provider Dispute Resolution portal at cms.gov/nosurprises.
- Submit your dispute online or by mail using the forms provided.
- Include a copy of your Good Faith Estimate and the bill you are disputing.
- There is a small administrative fee ($25 as of 2026) that may be waived for financial hardship.
During the dispute, the disputed portion of the bill cannot be sent to collections.
